A new Commonwealth Fund survey reveals that patients who received care from a medical home were less likely to report medical errors and care coordination failures. Patients connected with medical homes also had better relationships with their doctors and rated their care more highly. The survey included 18,000 sicker adults in 11 countries and found that patients in the US had the highest rates of problems paying medical bills and not receiving needed care due to cost.
