5 steps to strategic SDOH integration

The Office of the National Coordinator of Health IT has created a Social Determinants of Health (SDOH) toolkit to help healthcare providers understand key social factors impacting patient health. The Georgia Health Information Network is adopting this approach and emphasizing data exchange beyond clinical data for holistic patient care. They will present their strategy at the Enhancing Whole-Person Care Through Clinical and Social Data Integration session at HIMSS24 in Orlando, Florida.
