OIG flags active medication list issues in VA EHR

The US Department of Veterans Affairs (VA) has raised concerns over inaccuracies in veterans’ medication data due to an issue with the Oracle-Cerner electronic health record (EHR) system. The Office of Inspector General reported to the House Veterans Committee that the patient safety risk is related to medication ordering and dispensing. The EHR issue, discovered as a result of a reported prescription backlog, has affected around 120,000 veterans so far.
