ChristianaCare to expand medication adherence program

ChristianaCare’s digital medication adherence platform has significantly improved outcomes for Black cardiology patients with hypertension and heart failure. Developed with Story Health, the platform has shown a two to three times improvement in some key metrics. The health system plans to expand the platform’s access to more patients to help address issues of health equity. The platform also addresses social determinants of health and offers deeper, more accessible relationships between patients and clinicians.
