Large incidents of unauthorized access also top the list, with 70 such incidents affecting over 15 million people. The largest unauthorized access breach affected 13.4 million people by health plan Kaiser Foundation. Business associates are present in 141 breaches, representing 40% of total breaches. These figures are expected to increase dramatically due to unreported mega breaches.
