Breach at Verity Health System exposes data of more than 10,000 patients

The website of Verity Medical Foundation-San Jose Medical Group was hacked, exposing the data of 10,164 patients. The breach occurred from October 2015 until it was discovered in January 2018. The breached data included names, dates of birth, medical record numbers, addresses, emails, phone numbers, and the last four digits of credit card numbers. Verity Health has taken steps to secure the site and is providing free credit monitoring services to affected patients.
