Bridging gaps in post-acute care requires technology and interoperability

Longevity Health Plans Chief Population Officer Heidi Wold has called for technology and interoperability improvements in senior living settings, bringing them up to the level of hospitals. Doing so could mean simple medical devices can be used for remote patient monitoring. Interoperability across a common platform between hospitals and care homes could also speed up the exchange of medical information. These changes could trigger better patient outcomes and cost controls.
