Evara Health is tackling chronic conditions and saving lives with RPM

US healthcare provider Evara Health has selected Health Recovery Solutions as the vendor for its remote patient monitoring (RPM) programme for elderly and transportation-challenged patients. The RPM service, funded by a Federal Communications Commission telehealth grant, allows ongoing health monitoring of patients discharged from hospital or currently in the care of the Medical Home@Home service. The service has led to improvements in the treatment of high blood pressure and glucose rates.
