Findhelp and Pennsylvania HIEs to build SDOH navigator

Pennsylvania’s Department of Human Services is collaborating with the state’s health information exchanges, and Findhelp, a closed-loop referral platform vendor, to build an online social care tool called PA Navigate. The platform will allow Pennsylvanians, care agencies, and government bodies to find and track social services. It will offer healthcare providers a comprehensive view of an individual’s social care history and status. Over 750,000 people have used the platform in the past five years.
