Georgia partners with Aetna Health for Georgia Unity, a social data platform for whole health integration

Georgia Health Information Network used social determinants of health to close gaps in the state’s rural communities using its information exchange network, Georgia Unity. Developed in collaboration with local partners, the technology uses referrals to target individuals in need. Despite challenges like limited tech use and financial constraints, the project resulted in annual savings of $11,248 per person and 2,000 community health resource referrals.
