Going beyond SDOH to reduce readmissions

According to a recent study by the Office of the National Coordinator for Healthcare IT, 83% of U.S. acute care hospitals collect at least some social determinants of health (SDOH) data, with 54% doing this regularly. Furthermore, research by NorthShore Division of Cardiology and Laguna Health suggests using this data along with ‘life context’ can reduce readmission costs. Applying technological solutions like artificial intelligence could improve interventions by detecting recovery barriers, facilitate documentation, and ensure consistency.
