A UK NHS trust responsible for a psychiatric hospital admitted health and safety violations after one of its patients, Myha Grant, stabbed two of its employees and set fire to the ward. The Oxleas NHS Foundation Trust acknowledged two breaches of the Health and Safety at Work Act for not handling the risks associated with inpatients and not providing necessary health and safety information.
