UCLA chosen to lead dementia care under new Medicare alternative payment model

UCLA has partnered with the Centers for Medicare & Medicaid Services to create a new payment model for dementia care under the GUIDE model. The program provides dementia patients and their caregivers with resources like care coordination, round-the-clock access to health team members, care navigation, and caregiver respite services. The aim is to improve care access and coordination for dementia patients, and reduce caregiver strain to enable more home-based, community care.
