A report by think tank Parliament Street found that the UK National Health Service (NHS) lost nearly 10,000 patient records last year, with 68 NHS trusts misplacing records. University Hospital Birmingham was the worst offender, reporting 3,179 missing records. The report also highlighted the need for the NHS to improve in several areas, including abolishing handwritten notes in hospitals and introducing a patient identity protocol to protect patient documents.
